When a Retiree passes away, his or her spouse is entitled to 80% of their benefit. The members benefit ends and is transferred to the spouse In this process, a packet is sent from the State Retirement Agency to the Retirement Coordinator for the spouse to complete. It includes:
- Beneficiary Claim form.
- Direct Deposit (the spouse has to have the bank complete section II) to change the account over to the spouse even if it is the same account.
- Federal and State Withholding forms.
- Health Benefits application (this again changes the benefit to the spouse).
These forms must be completed and returned to the State Retirement Agency along with a Certified copy of the Death Certificate, a copy of the spouse’s Social Security card and proof of Birth (Drivers License/Birth Certificate) and a copy of the Marriage Certificate.
